The easiest way to create a sign-up sheet is to select a ready-made template from this page. You can then download it in any of four formats: Word, Excel, editable PDF or as an image file. The format that you choose will depend on how you want to use the form and how much you want to edit it. An image format cannot be edited at all whereas the Word form can be customized to your needs.
